Determinants of Caeserean Delivery in Ayder Referral Hospital, Mekelle, Tigray, Ethiopia 2015: A Case Control Study

Abstract


Caesarean delivery rates have been increasing around the world from time to time, with current rates in developing countries comparable to other industrialized nations. International concern over such increases has prompted the World Health Organization to suggest that caesarean section rates should not exceed 15%. A study conducted in Ayder Referral Hospital indicated that the rate of caesarean section was 21.90%. The aim of the study was to assess the determinant of caesarian delivery in Ayder Referral Hospital, Mekelle, Tigray, Ethiopia 2015. A retrospective case control study was conducted. The cases were all mothers delivered by caesarean section and controls were all mothers delivered by spontaneous vertex deliveries in the same time period at Ayder Referral Hospital. The case to control ratio was 1 to 2. Systematic random sampling technique was used from health management information system of Ayder Referral Hospital. Data was entered, cleaned and analyzed by SPSS version 20 statistical software. Finally, frequencies, percentages, mean, standard deviation, bivariate analysis and multivariate logistic regression were computed. The odds of ANC third visits were 3.8 times (AOR = 3.75, 95% CI; 1.466–9.588) more likely to undergo CS compared to those who have ANC first visit. Women who were not followed by partograph during labor were 51% more likely (AOR = 0.51, 95% CI; 0.275–0.935) to deliver through caesarean section compared to those who was followed by partograph during labor. Mothers who have not previous caesarean section were 93% (AOR = 0.07, 95% CI; 0.018–0.310) less likely to have caesarean section compared those who have previous caesarean section. Antenatal care, previous CS, length of stay in days, sex of service offered clinician and partograph has significant association with caesarean section.
